BenefitDescription
401 (k) Savings Plan FDA currently matches 100% of allowed contributions up to 5% of your income. Loans are available as described in the plan description. You can begin to participate in the plan after you have completed 90 days of continuous employment prior to the Open Enrollment period. You will be notified when the Open Enrollment period is in order to sign up for this benefit. You may roll over funds immediately into the plan; no waiting period is required. A six-year vesting schedule applies to the employer's contributions. All participants must be 21 years of age or older.
Health Insurance Health lnsurance is offered. Spouse and dependent coverage is available.
Dental InsuranceThis dental plan allows you to go to a dentist of your choice. Spouse and dependent coverage is available.
Short Term Disability FDA pays 25% of this coverage after you have been employed for 60 days. This coverage has a seven-day waiting period and covers 50% (up to a maximum of $1,000) of your base salary for 90 days.
Long Term Disability FDA pays for 100% of your long term disability insurance after you have been employed for 60 days. This coverage has a 90-day waiting period and covers 66.67% of your base salary and integrates with Social Security to age 65.
Group Life Insurance Employees are eligible to receive $50,000 of life insurance at no charge after 30 days of service.
Voluntary Life InsuranceAdditional life insurance may be purchased for the employee and family members. The employee is responsible for the entire cost of this coverage.
Section 125 Plan The Section 125 Plan provides tax savings by reducing employee medical premiums from your gross salary prior to calculation of federal income and social security taxes, as allowed under Internal Revenue Code (IRC) Section 125.
Holidays The following paid holidays will be observed: New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Day after Thanksgiving and Christmas Day. Full-time employees will receive eight hours pay per holiday.
Personal Time Off (PTO) Employees will earn 120 hours per year for the first five years of service, 160 hours per year after five years of continuous service, 200 hours per year after 10 years of continuous service, and 240 hours after 20 years of continuous service. These days accrue each pay period.
Tuition Reimbursement Courses relevant to the employee's position will be reimbursed after satisfactory completion of the course. Books are not included in this benefit. Must be pre-approved.
Professional Registration Fees All pre-approved required fees will be paid by FDA.
Professional Association FeesOne professional association's annual dues will be reimbursed each year.
4-1/2 day (40 Hour) WorkweekHome office employees are able to work this weekly schedule. Field employees will work the schedule set by the client.
